AI SAFETY STORY

Perspective from a Patient/Nurse/Nurse Educator

Last year, I was injured in an accident and went to a follow-up appointment with my physician. During the visit, an AI-powered tool recorded our conversation. We discussed my symptoms, treatment plan, imaging studies, medications, and follow-up care, and it generated the clinical note.

Since the 21st Century Cures Act allows patients to read clinical notes, I reviewed the note through my patient portal.

What I found was concerning!

Important details about my symptoms were inaccurate. Other information relevant to my injury claim was omitted. The note appeared complete, but it did not accurately reflect what had occurred during the appointment.

Although the AI generated the documentation, my physician remained professionally and legally accountable for the contents of that record when signing the note.

That experience changed the way I think about
AI in healthcare.

As a bedside nurse and clinical educator with 33 years of experience, I see AI embedded in clinical workflows, documentation, and decision-support systems. While these tools can be valuable, they are not flawless—and nurses are still responsible for the care they provide.

That's why I developed the CHECK-AI™ Framework—for myself, my colleagues, and every nurse navigating AI-supported care:
